The Student Formation Program of the Student Affairs Department (StAD) of Letran- Calamba is designed to manage student-related matters concerning student discipline. It includes resolution of grievance and imposition of sanctions to students found guilty of violation of school rules and regulations after due process. According to the Student Handbook, there is a process in issuing referrals to students. This research aimed to help the Student Affairs Department to simplify the difficult and time-consuming task of tracking student violations and to help improve school discipline by ensuring that students are held accountable for their actions. A student discipline referral system will help manage student discipline referrals and to view individual student's discipline records in the system. The researchers used the descriptive research method in gathering the feedback on the developed system from the system’s supposed users, specifically the StAD personnel. The data indicated that the system developed could help the Student Affairs Department to document and track disciplinary incidents, maintain related records, and produce reports, notices, forms, and other documents easily and efficiently. The researchers recommended to the future researchers to improve the system into a web and mobile application for easier user’s access and to facilitate the issuance of student discipline referrals. Moreover, adding prescriptive analytics and upgrading the data visualization of the system may be considered by the other future researchers to further enhance the current systems’ capabilities.
