Factors Leading to Conversion to Open Cholecystectomy in Laparoscopic Cholecystectomy: A Single Center Experience

Abstract:

Objective: To determine the current rate of conversion from laparoscopic cholecystectomy to open cholecystectomy as well as the various factors leading to its conversion at the Manila Central University FDTMF Hospital. Methods: This study is a cross-sectional analytical study of patients who underwent cholecystectomy at the Manila Central University Hospital between 2019 to 2020. The current rate of conversion from laparoscopic cholecystectomy to open cholecystectomy as well as the various factors leading to its conversion were analyzed. Results: Out of 170 participants, 3 patients were converted to open cholecystectomy. The reason of conversion in these patients were the presence of a difficult gallbladder intraoperatively namely perforated gallbladder, gangrene and empyema, and hydrops of the gallbladder. All 3 cases did not have any ultrasound findings which would have pointed to a difficult gallbladder. Conclusion: MCU FDTMF has a significantly low rate of conversion from laparoscopic cholecystectomy to open cholecystectomy. This may be attributed to highly skilled surgeons affiliated with the institution. A high clinical index of suspicion for difficult cases and good patient selection would also have committed to this low conversion rate. The significant factor of conversion cannot be determined easily using imaging and overall clinical status of the patient
